Pool Registration - Westwood Village Unit Owners Association

Please fill out this form to register for the pool at the community clubhouse. Registration is required in advance of pool use.

Residents 18 and older must complete a separate registration and sign their own waiver and consent form. An adult can register and sign the waiver and consent form on behalf of their children 17 and younger.

Liability Waiver/Informed Consent Form

I agree and consent to the following:

I am voluntarily using the pool and participating in swimming and other pool related activities at the pool located at Westwood Village and am fully aware of the risks and hazards involved.

I understand that it is my responsibility to consult with a physician prior to and regarding my participation in swimming and other pool related activities. I represent and warrant that I have no medical condition that would prevent my participation in such activities.

I agree to assume full responsibility for any risks, injuries, or damage which I might incur as a result of using the pool. Such injuries may include, but are not limited to, contraction of COVID-19 and/or other viruses, heart attacks, muscle strains, muscle pulls, muscle tears, broken bones, or any other illness or soreness that I may incur, including death.

I knowingly, voluntarily, and expressly waive any claim I may have against The Townhomes at Westwood Village and/or the Westwood Village Unit Owners Association for injury or damages that I may sustain as a result of the use of the pool including, but not limited to, claims that arise and/or result from the negligence of The Townhomes at Westwood Village and/or the Westwood Village Unit Owners Association and/or the agents, employees, officers or members of such Associations. I expressly acknowledge that my use of the pool may lead to contraction of COVID-19.

I hereby release and agree to hold harmless The Townhomes at Westwood Village and/or the Westwood Village Unit Owners Association, and their respective directors, officers, employees, members, residents and agents for any claims, liabilities, injuries, damages to my person or property, direct or indirect, including but not limited to costs and attorney’s fees, arising from, caused by, or the result of my use of the pool.

I further hereby agree that I am liable for any and all claims, liabilities, injuries, damages to persons or property, direct or indirect, including but not limited to costs or attorneys’ fees, arising from, or as a result of my use of the pool. I agree to reimburse the Associations for any such claims and/or liabilities, injuries, damages to persons or property, including but not limited to, costs and attorneys’ fees.

I have read the above waiver and release of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above. If I am waiving and releasing on behalf of a minor, it is noted below.
